Do you accept new patients?

Yes we have some doctors accepting new patients. There is a new patient registration form the details of which need to be filled out. You can download the patient registration form by clicking on the link on our home page. Please ask reception staff for more details as to which Doctors are accepting new patients.

Do you do home visits?

Yes we do home visits at the discretion of the doctor concerned.

Can I request for prescriptions/ referrals over the phone?

From 1st of August 2016 you now require an appointment for all repeat prescriptions, wherever possible please arrange an appointment in advance.

For specialist referrals please contact our friendly reception staff.

Can I get my results over the phone?

We put the request through to your doctor and only give those results that your doctor has authorised to be given over the phone.

How do I get my medical history sent over from previous clinic? / How can my history be sent to another clinic?

There is a form to be filled in by you requesting for the same & giving authority for us to collect this information, and for your previous clinic to release this information. In the same way your history can be sent to another clinic.

How do I claim my Medicare rebate?

This can be done on the day at the clinic itself.  If you have a savings or cheque options on a credit or debit card, or if we or Medicare have your bank details.  Alternatively you can also go in personally to a Medicare office and claim.

How do I book an appointment to see a nurse?

Call the clinic on 5472 1844 and one of our friendly reception staff will help you.

If you are calling for a Care Plan or Health Assessment the request is put in and the nurse will get in touch with you.

What is a My Health Record

My Health Record is a national digital health record system.  Having a My Health Record means your important health information like allergies, medical conditions and treatments, medicine details, test and scan reports can be digitally available in one place.  This allows healthcare providers such as doctors, specialists and hospital staff to access your health information online anywhere at any time when they need to, for example in an accident or emergency.  If you have a Medicare of DVA card you will automatically get a My Health Record unless you decide to opt out.  To opt out you can call the help line on 1800 723 471 or via their website www.myhealthrecord.gov.au before 31st January 2019.

What happens if I miss an appointment?

There is a fee that may be charged for missed appointments. Reminders can be sent if you wish, just request the reception staff to place you on the reminder list.

Are translating and interpreting services available?

If an interpreter is required for your appointment please let reception staff know when you make the appointment.

How is my confidentiality and privacy managed?

Botanical Gardens Health takes your privacy seriously.  Your medical and personal information is strictly confidential and we take great care to maintain its privacy.  The confidentiality of your medical information is regulated by the Health Records Act 2001 (Vic) and the Privacy Act 1998.  Please speak to our reception staff if you would like a copy of our privacy policy.

Are you open on code red (catastrophic) fire days?

When a code red (catastrophic) fire day is declared by the CFA Botanical Gardens Health will see emergencies only.

How do I change my personal information?

Please speak to our reception staff if you need to update your contact details.  It is important we have your correct address and contact numbers if we ever need to contact you.
